The Disclosure & Barring Service Process & Self Declaration Form

UKA takes its responsibilities towards the welfare of young people in Athletics very seriously and requires everyone in athletics who has significant contact with children to have an Enhanced Disclosure Barring Service Check.

UKA have now moved to an online system for DBS Applications, please follow the link below for further instructions.

1.   Log in at www.englandathletics.org/myprofile to check your DBS status, please ensure your contact details are correct. Your Username for your portal is your UKA License Number shown on your UKA card.

If you have any queries about accessing your portal please contact England Athletics Membership Services on 0121 347 6554.

If you encounter any problems whilst completing the online form or require any technical assistance please contact the UKA Welfare Team on dbs@uka.org.uk outlining your problem and a member of the team will contact you to assist in resolving the issue.

You will require a Self-Declaration form if you are:

U-16 years of age.

Parent Helpers.

General club volunteers that do not have sole supervisory responsibilities for U-18's.

Unqualified coaches/officials that are assisting to gain experience prior to accessing a formal course.
